Received without fixative/in formalin, labeled _____ and “_____”, is a portion of fibroadipose tissue measuring __ x __ x __ cm, with an attached nipple measuring __ cm in diameter and __ cm in height. The specimen is oriented by a long suture (e.g. superior) and a short suture (e.g. lateral). No abnormalities are noted on the nipple skin. The margins are inked as follows: black = deep, blue = superior, green = inferior, yellow = anterior, red = medial and lateral. There is blue dye on the cut surface corresponding to a __ cm duct which is contiguous with the nipple. __ cm deep to the nipple and attached to the inner lining of this duct is a __cm _____ (color) papillary lesion. The breast tissue surrounding the lesion is fibrotic. The lesion is __ cm from the _____ margin, __ cm from the _____ margin, __ cm from the _____ margin, __ cm from the _____ margin, __ cm from the _____ margin, and __ cm from the _____ margin. The breast tissue in the rest of the specimen is predominantly fatty. No additional lesions are identified. Representatively submitted as follows:
