Identify the components of the specimen: penis (partial vs. total), scrotum, testis, spermatic cord, inguinal lymph nodes. If lymph nodes are present, dissect them in the unfixed state.
Measure the overall specimen and each individual component (three dimensions).
Describe (color, borders, ulcer, mass) and measure lesions on the external surface; determine the site of involvement (glans, prepuce, shaft, scrotal skin) and distance to the resection margin.
Describe the uninvolved tissue.
Examine and ink the resection margin (skin, soft tissue, urethra).
Incise the specimen in half sagittally, dividing the urethra in two, while maintaining specimen orientation.
Make additional incisions through lesions and determine their depth of invasion and extension to underlying structures (corpora cavernosa – right vs. left, corpus spongiosum, urethra).
Tumor: five sections, including adjacent uninvolved tissue in one or two sections and area of deepest invasion in two sections. You may need to take more sections to document infiltration of adjacent structures.
Additional lesions: up to three sections per lesion, depending on size.
Resection margin: cross-section of entire penis margin. It is usually submitted en face (divided in half and placed in two cassettes); however, if the lesion is very close to the margin, ink the margin, remove as a complete cross-section, and section perpendicularly. If the resection margin also includes scrotum, take margin sections closest to the tumor.
