•    Transurethral resection of the prostate is performed for hyperplasia.

• Weigh and measure (three dimensions) the fragments in aggregate.  Also measure the diameter of the largest fragment.
• Examine and describe (color, rubbery, firm) the fragments.  Pay close attention to any areas which appear yellow and firm, as these could represent carcinoma.
• It is not necessary to cut the fragments unless they are too large to fit into a cassette.

• If the weight is 12 g or less, submit in toto, approximately 12 block.
• Submit one additional block for each additional five grams of tissue. (Example: 23 g specimen= 23 g – 12 g= 11 g (2 additional blocks))
• If carcinoma is identified in the histologic sections, submit any remaining tissue.

Received without fixative/in formalin, labeled _____ and “_____”, are multiple gray-tan, rubbery soft tissue fragments weighing __ g and measuring __ x __ x __ cm in aggregate.  The largest fragment is __ cm in diameter and has a nodular, firm, yellow area on its cut surface, __ cm in greatest dimension.  Entirely submitted in #__ - #__  (largest fragment in cassette #__ ).
